ABSTRACT

OBJECTIVES: To study the organization and practices in nosocomial infection (NI) control in regional and provincial hospitals. MATERIAL AND METHOD: Data were collected by questionnaires answered by chairpersons of infection control committees (ICC) infection control nurses (ICNs) and link nurses in regional and provincial hospitals. RESULTS: From April to June 2002, sets of questionnaires were sent to all chair persons of regional and provincial hospitals, 120 infection control nurses and 57 link nurses. Of 92 hospitals, 78.3% of chairpersons and all ICNs and link nurses responded. The ICC were chaired by the directors or deputy directors in 26.4% and by doctors in various specialties in the remaining. Among ICNs, 14.2% had never attended a course in infection control and 62.5% had less than 6 years experience. Lack of support from administrators, budget, co-operation from medical personnel and ICN position were the main problems in the IC program. CONCLUSION: Nosocomial infection control in regional and provincial hospitals in Thailand needs more support from administrators and more co-operation from medical personnel.

ABSTRACT

BACKGROUND: Telepathology allows quick and timely access to an expert opinion, no matter where the patient is located. We analysed the experience with the use of telepathology between a tertiary cancer centre and a rural cancer hospital. METHODS: Ninety-three cases were analysed in which static telepathology was used to obtain a consultation between Tata Memorial Centre and Nargis Dutt Memorial Cancer Hospital at Barshi, a rural area. RESULTS: Successful teleconsultation was achieved in all cases. A diagnosis was offered in 92 cases (98.9%) and was deferred in 1 case (1.1%). Complete concordance, clinically unimportant minor discrepancy and hedged diagnosis were obtained in 83 cases (90.2%). Major discrepancies were encountered in 9 cases (9.7%). The number of images per case ranged from 3 to 27 (average: 7 images). Images were of diagnosable quality in 89.2% of cases. Most of the cases (77.4%) were reported within 3 days; 32.2% were reported within 8 hours (a single working day) and 45.1% within 1-3 days. CONCLUSION: Telemedicine can be effectively used to bridge the gap between medically underprivileged, geographically distant rural areas and advanced centres using the static store and forward methodology.

ABSTRACT

Bed management is one of the important activities for efficient hospital management. The present study on evaluation of bed management in a rural hospital revealed that the total bed capacity could not be utilised. The turnover rate, turnover interval, bed occupancy rate and average length of stay were closely corroborating.

Describe los aspectos administrativos del Hospital San Antonio y dentro de este contexto, la ubicacion del Departamento de Enfermeria y las funciones del personal. Presenta las areas y servicios de salud del hospital y los programas de atencion al medio ambiente y a las personas. Finalmente, hace una evaluacion de la experiencia como estudiante de enfermeria en la zona rural.
